Many locations in Fallout Online are unguarded, that is no semblance of law and order exists, or very little. Every location north of the Shady Sands/the pit line is unguarded except the Klamath mines, Vault City, and VC's train station. South of that line, Vault 15, the Cathedral, Necropolis, Waterworks, the Warehouse, and The Glow are unguarded. (Of these, only the Cathedral and Necropolis are occupied, and the Ghouls of Necropolis are hostile to everyone anyways.)

While the towns are dangerous for PC's, they're safe enough for NPC's, except New Reno. Attacking an NPC can and generally will result in a violent response. Even in New Reno, an attack on members of one faction (one of the families or prostitutes/drug dealers/junkies) can result in an attack by members of that faction but likely will be ignored by everyone else (in some cases, rewarded). In the southern locations, combat by PC's will be joined by the locals (if there are any), but only Necropolis will result in active, preemptive attacks by the locals (at the main entrance).
